How do I register for the Accessibility Service? What information/documentation do I need to provide?

The Process

In order to register you need to have your supporting documentation (specialist’s report or Health Practitioner’s Report (HPR)) ready to upload. Your health practitioner can complete the HPR found at the link below.

1.       Go to https://students.mq.edu.au/support/wellbeing/diversity-and-inclusion/accessibility-support

2.       Click on ‘Registration steps in detail’ and select “Registration form”

3.       Log-in with your student OneID.

4.       Follow the prompts and attach your documentation.

 

After you have registered, a member of the Accessibility Service will be in contact with you within 2 business days to confirm registration and to book your disability assessment.
